After Piett is promoted from captain to admiral in The Empire Strikes Back, his rank insignia is upgraded from three red and blue bars to six. However, throughout the events that occur in Return of the Jedi, his insignia is three red and blue bars, even though Piett still holds the rank of admiral.

K. M. Hoffman's comment: Hmm... maybe the Empire was trying to hide his importance from the rest of the world?

Grand Admiral's note: Actually, this was a major continuity mistake on the part of those involved in costuming the Imperial Officers in Return of the Jedi! It would seem that in ROTJ, ALL Imperial Officers have exactly the same rank indignia on their uniforms - Admiral Piett and Moff Jerjerrod included.
