Grand Admiral Hayden, this is Grand Moff Neal Fisher COP Member #140.  According to Star Wars:  Behind the Magic Admiral Piett actually obtained the rank of Head Grand Admiral, if such a position existed.  The following is from Behind the Magic,

"Admiral Piett

One of the few Imperial officers who never incurred Darth Vader's wrath, and the able commander of the ominous Executor throughout much of the Galactic Civil War.  Piett who received several promotions after his predecessors enraged Vader, remained a confident and ruthless leader despite the demands of his position.  His competence was rewarded shortly before the Battle of Endor, when he was given command of the entire Imperial fleet.  He served with characteristic dedication until he met a grisly demise aboard the Executor."

The most significant aspect of this passage is that Piett was given command of the ENTIRE Imperial Fleet.  This position was formerly held by Lord Vader after the Battle of Yavin, since Piett was given the position of Supreme Fleet Commander he must of been given the rank of Grand Admiral-in-Chief.  Since there were supposedly twelve grand admirals appointed by the Emperor to directly control the Imperial Forces it stands to reason that Piett was installed as the Chairman of the Grand Admiralty since he was single handily given command of all forces.  This would make Piett the third most powerful man in the Galactic Empire, subsequent only to the Lord Vader and Emperor Palpatine, this is appropriate because Piett is a tactical genius as well as Force Sensitive, according to his SW Customizable Card Game card.  The passage also says that Piett was a ruthless leader, this should be interpreted as being ruthless in pursuing his duty to win the war, and not ruthless as in cruel to his crew or prisoners which Piett was seldom if ever guilty of.  The passage also suggest that Piett met a "grisly demise" this doesn't imply that Piett is necessarily dead, it only implies that Piett lost his powerful position at Endor, his career may have met a demise although the man obviously did not.  The  passage also states that he was ADMIRAL Piett as opposed to Captain which many texts make the mistake of naming him.  The evident question which this passage brings up is why didn't Piett have the uniform of a Grand Admiral if that was his new rank.  There are several explanations for this, (1) the costume department made the error of awarding every gray-clad Imperial officer the rank of captain, (2) Piett obtained his rank shortly before Endor and the uniform hadn't arrived yet or he hadn't returned to Coruscant for the ceremony that accompanied his promotion, and (3) Piett probably didn't want to make his fellow officers over confident in for the battle ahead a fact that many past leaders made (such as Grand Moff Tarkin and that scum Admiral Ozzel) in assuming that the rebels were not a danger.
