A guide to the Star Wars universe describes Admiral Piett as:

Captain (I think they mean Admiral) Piett-

Captain (Again) Piett served as first officer (Again) on the flagship Executor. He assisted Admiral Ozzel in watching over the crew, as well as helping to direct the entire fleet assigned to the flagship. He was promoted to Admiral, by Vader, when Ozzel made a fatal mistake. He was given the flagship and was cloned shortly afterwords. His clone remained in comand of Executor until it was lost in batle durring the battle of Endor. [Apeared in Empire Strikes Back and Return of the Jedi]

Note: The refrences of him being captain and first officer are before the great day when Ozzel died.
